Serum homoarginine concentrations were assessed in 3,305 LURIC participants followed for a median of 9.9 years to evaluate associations with fatal stroke and prior cerebrovascular disease. Each standard deviation increment in homoarginine corresponded to an odds ratio of 0.52 (95% CI 0.37-0.73, p < 0.001) for fatal stroke, remaining significant after multivariable adjustment (OR 0.62, p = 0.014). Low homoarginine thus represents a novel vascular risk factor with potential relevance for stroke risk stratification and prevention.
