Serum 25(OH)D concentrations and mortality were examined in 444 LURIC participants with an estimated glomerular filtration rate below 60 mL/min/1.73 m2. Over a median follow-up of 9.4 years, severely vitamin D-deficient patients (25(OH)D <10 ng/mL) showed adjusted hazard ratios of 3.79 for all-cause and 5.61 for cardiovascular mortality compared with vitamin D-sufficient patients. The findings support correcting vitamin D deficiency in chronic kidney disease, though randomised trials are needed to establish whether supplementation improves survival.
