Among 3,141 LURIC participants followed for cardiovascular mortality, HDL cholesterol was inversely associated with outcome (p = 0.009) but was protective only in those without coronary artery disease (CAD; HR = 0.37, highest vs. lowest tertile, p = 0.005), not in stable or unstable CAD patients (interaction p = 0.007). Replicated in AtheroGene and ESTHER cohorts, these results suggest HDL cholesterol has limited value for risk stratification in CAD patients.
