The associations of serum amyloid A (SAA) and HDL-cholesterol (HDL-C) with mortality were examined in 3310 LURIC patients and validated in the 4D (n=1255) and KORA S4 (n=4027) cohorts. Higher HDL-C predicted lower all-cause and cardiovascular mortality when SAA was low, but was associated with increased mortality when SAA was high. These results indicate that SAA modifies the cardioprotective properties of HDL and that concomitant SAA measurement provides a clinically applicable surrogate for HDL vascular functionality.
