Fibroblast growth factor 23 (FGF-23) was examined as a mortality predictor in 980 LURIC heart failure patients (511 with reduced ejection fraction [HFrEF], 469 with preserved) over a median follow-up of 8.6 years. FGF-23 was independently associated with mortality in HFrEF (adjusted hazard ratio per 1-SD increase 1.30, 95% CI 1.14-1.48, P<0.001) but not in preserved ejection fraction, an association validated externally, suggesting distinct pathophysiologic roles across heart failure phenotypes.
