In 2290 LURIC participants followed for a median of 10.0 years, with 434 cardiovascular deaths, HDL particle subclasses were quantified by nuclear magnetic resonance spectroscopy. Total HDL particle concentration was inversely associated with cardiovascular mortality (HR third vs first tertile = 0.55, p < 0.001), an association driven primarily by small HDL particles and independent of coronary artery disease. HDL particle concentration outperformed HDL cholesterol in multivariable mortality prediction.
