A genetic score of two intronic SLC5A2 SNPs was associated with lower heart failure risk in the UK Biobank (n = 416,737; OR 0.97, P = 0.016), with validation in LURIC (n = 3,316). HbA1c, HDL cholesterol, uric acid, blood pressure, and body composition mediated 35% of this effect; no associations with atherosclerotic outcomes were observed. These findings suggest SGLT2 affects heart failure risk through multiple metabolic pathways independent of diabetes or coronary artery disease.
