In 3,291 LURIC participants followed for 9.9 years (621 cardiovascular deaths), elevated LDLapoB/LDLC ratios were independently associated with increased cardiovascular mortality (fourth vs. first quartile: hazard ratio = 2.07, 95% CI 1.53-2.79) and with higher concentrations of the smallest LDL subclasses (LDL5 and LDL6, p < 0.001), replicated in 1,660 independent participants. The ratio provides a practical surrogate for estimating small, dense LDL-related atherogenic risk from routine laboratory measurements.
