Interleukin-6 and hsCRP were analysed in 1,086 LURIC patients with heart failure (506 HFpEF; 580 HFrEF) to assess their prognostic value for cardiovascular mortality. Elevated IL-6 predicted CV mortality in HFpEF (HR 1.5, 95% CI 1.1-2.2, P = 0.018) but not HFrEF, and homozygous carriers of the IL-6 receptor variant p.Asp358Ala showed lower NT-proBNP specifically in HFpEF. These results suggest that IL-6 signalling plays a distinct pathophysiological role in HFpEF.
