Among 3316 LURIC participants followed for a median of 9.9 years, 603 cardiovascular deaths occurred; the highest versus lowest Fatty Liver Index tertile carried an elevated mortality risk (HR 1.39, 95% CI 1.02-1.89), amplified in those with elevated hsCRP (HR 1.58) or IL-6 (HR 1.93). Systemic inflammation thus modifies the steatosis-cardiovascular mortality association, indicating that both metabolic and inflammatory markers should be considered jointly in risk assessment.
