Associations between pulse pressure (PP) and electrolytes, hormones, and clinical outcomes were examined in 3,316 LURIC participants. Potassium, phosphate, renin, and vitamin D concentrations fell with rising PP, whereas sodium-potassium and aldosterone-renin ratios rose. Women faced a greater cardiovascular mortality risk per SD of PP (OR 1.57) and men a higher risk of reduced eGFR (OR 1.59), indicating sex-specific pathways linking arterial stiffness to end-organ damage.
