Plasma asymmetrical dimethylarginine (ADMA) was measured in 3,238 LURIC participants (2,543 with and 695 without angiographic coronary artery disease) and related to mortality over 5.45 years of median follow-up. The highest ADMA quartile showed adjusted hazard ratios of 1.87 for all-cause and 1.81 for cardiovascular mortality versus the lowest quartile. ADMA independently predicted mortality in CAD patients beyond established risk factors but not in those without angiographic disease.
