Cholestanol, lathosterol, campesterol, and sitosterol were measured in 2,440 LURIC participants to assess their association with coronary artery disease severity by the Friesinger Score. Higher cholestanol-to-cholesterol ratios were associated with greater severity (p = 0.006), while higher lathosterol-to-cholesterol ratios corresponded to lower severity (p < 0.001). The findings indicate a modest atherogenic effect of high cholesterol absorption relative to low synthesis, but do not support an independent atherogenic role of plasma plant sterols.
