Neopterin was measured in 2312 LURIC participants (1801 with and 511 without angiographic coronary artery disease) followed for a median of 8.0 years. In the highest neopterin quartile, unadjusted hazard ratios were 3.32 for all-cause and 3.84 for cardiovascular mortality versus the lowest quartile; associations persisted after multivariable adjustment, though NT-proBNP substantially attenuated them. Neopterin is an independent predictor of mortality in patients with or without stable coronary artery disease.
