Plant sterol markers of cholesterol synthesis and absorption were measured in 1,257 statin-free LURIC participants followed for a mean of 7.32 years. The highest lathosterol-to-cholesterol tertile was associated with lower all-cause (p = 0.001) and cardiovascular (p = 0.006) mortality, whereas the highest cholestanol-to-cholesterol tertile predicted increased all-cause (p < 0.001) and cardiovascular (p = 0.010) mortality, indicating that high absorption with low synthesis confers elevated mortality risk.
