Glucokinase-activating GCKR polymorphisms increase plasma levels of triglycerides and free fatty acids, but do not elevate cardiovascular risk in the Ludwigshafen Risk and Cardiovascular Health Study

Two glucokinase regulator protein polymorphisms (rs780094 and rs1260326) were genotyped in 3,500 LURIC patients to determine whether long-term genetic glucokinase activation elevates cardiovascular risk. Both variants significantly increased plasma triglyceride and VLDL triglyceride levels (p < 0.0001) and free fatty acid concentrations (p < 0.0078), yet showed no association with coronary stenosis, myocardial infarction, left ventricular hypertrophy, or hypertension. These findings suggest that pharmacological glucokinase activation is unlikely to confer excess cardiovascular risk through this lipid-raising mechanism.
