Serum homoarginine levels were assessed in 3,305 LURIC participants referred for coronary angiography and in 1,244 haemodialysis patients from the 4D study. In LURIC, patients in the lowest quartile (<1.85 micromol/L) had a four-fold higher cardiovascular mortality rate (hazard ratio 4.1, 95% CI 3.0-5.7) than those in the highest quartile over 7.7 years of median follow-up. Consistent findings across both cohorts indicate that homoarginine independently predicts mortality in high-risk patients.
