This review examined HDL’s role in lipid metabolism and cardiovascular protection, covering its participation in reverse cholesterol transport and its anti-oxidative, anti-inflammatory, and anti-thrombotic properties. The failure of the CETP inhibitor torcetrapib to reduce cardiovascular events despite raising HDL levels was attributed to off-target toxicity rather than the inhibition mechanism itself. A new generation of CETP inhibitors was positioned for evaluation in large clinical trials.
