Among 2,686 LURIC participants without prior diabetes followed for a mean of 7.54 years, HbA1c showed a J-shaped association with all-cause mortality; the multivariable-adjusted hazard ratio for HbA1c >=7.5% versus the 5.5-5.9% reference was 2.15 (95% CI 1.32-3.53). HbA1c remained independently predictive after adjustment for fasting glucose, whereas fasting glucose lost significance when HbA1c was included, supporting HbA1c as the superior prognostic marker in this high-cardiovascular-risk population.
