Among 2236 LURIC participants followed for a median of 7.7 years, lower global arginine bioavailability ratio (GABR) quartiles were associated with higher cardiovascular mortality; the lowest quartile carried a hazard ratio of 1.75 (95% CI 1.24-2.45) versus the highest. GABR correlated inversely with VCAM-1 (r = -0.301, p < 0.001) and ICAM-1 (r = -0.136, p < 0.001), suggesting impaired arginine bioavailability marks endothelial dysfunction and independently predicts cardiovascular death.
