Among 1,801 LURIC participants with the metabolic syndrome followed for a median of 7.7 years, optimal 25-hydroxyvitamin D levels (>=75 nmol/L) were associated with substantially reduced all-cause (HR 0.25) and cardiovascular mortality (HR 0.33) compared with severe deficiency (<25 nmol/L). Reductions were dose-dependent and extended to sudden cardiac death and heart failure. The findings support clinical trials of vitamin D supplementation in this high-risk group.
