Among 2,306 LURIC participants undergoing coronary angiography, C-peptide levels in the highest versus lowest tertile were independently associated with all-cause mortality (HR 1.46, p = 0.008) and cardiovascular mortality (HR 1.55, p = 0.022) over a mean 7.6-year follow-up. Higher C-peptide was also associated with markers of endothelial dysfunction and more severe coronary lesions, implicating C-peptide as an independent predictor of atherogenesis.
