Associations between APOE genotype, smoking status, and mortality were analysed in 3,263 LURIC participants undergoing coronary angiography. Carriage of the epsilon-4 allele significantly increased total and cardiovascular mortality in current smokers (HR 2.95, 95% CI 2.10-4.17 vs. never-smokers; interaction HR 1.66, 95% CI 1.04-2.64) but not in ex-smokers, suggesting that smoking cessation may attenuate the excess cardiovascular risk attributable to the epsilon-4 allele.
