Among 1275 LURIC patients with stable coronary artery disease followed for a median of 9.8 years, 135 novel biomarkers were screened to extend the existing VILCAD risk score. A nine-variable BIO-VILCAD model comprising age, sex, LVEF, heart rate, NT-proBNP, cystatin C, renin, 25OH-vitamin D3 and HbA1c achieved a significantly improved C-statistic (0.78 vs. 0.73, p = 0.035) and net reclassification index of 14.9% (p < 0.001), suggesting that biomarker-augmented prediction enables more personalised management of stable CAD patients.
