Von Willebrand factor (vWF) was evaluated as a prognostic biomarker in 457 LURIC patients with heart failure with preserved ejection fraction (HFpEF), 40% of whom died over a median 9.7-year follow-up. vWF independently predicted all-cause mortality (adjusted HR per 1 SD 1.22, 95% CI 1.05-1.42, P=0.001) and improved risk prediction beyond NT-proBNP (C-statistic 0.65 vs 0.63, P=0.004; net reclassification index 37.6%), supporting endothelial dysfunction as a pathophysiological mediator in HFpEF.
