Serum cotinine, a major nicotine metabolite, was measured by radioimmunoassay in 840 LURIC participants to evaluate its utility for mortality risk prediction. Cotinine and pack-years both associated significantly with mortality in adjusted Cox models, with hazard ratios of 1.30 and 1.26, respectively, comparing the third to the first tertile. As an objectively measurable parameter, cotinine may be preferable to self-reported smoking for cardiovascular risk stratification.
