Renin-angiotensin-aldosterone system (RAAS) parameters were measured in 3316 LURIC participants, comparing 777 active smokers with 1178 lifetime non-smokers over a median 10-year follow-up. Renin and angiotensin-II were independently associated with all-cause mortality in both groups, whereas angiotensin-I was associated with reduced mortality risk only in non-smokers (HR 0.69, 95% CI 0.53-0.89). These findings indicate that elevated renin and angiotensin-II are independent predictors of mortality regardless of smoking status.
