Serum uromodulin was measured in 3057 LURIC participants referred for coronary angiography. Higher concentrations were associated with lower prevalence of hypertension, diabetes mellitus, and heart failure, and reduced 10-year mortality risk (HR 0.57, 95% CI 0.45-0.73 for Q4 vs Q1), independent of eGFR and stronger after adjustment for the rs12917707 UMOD genotype. Adding serum uromodulin to established risk scores improved cardiovascular risk prediction.
