Serum symmetric dimethylarginine (SDMA) and HDL cholesterol (HDL-C) were assessed in 3310 LURIC participants over a median follow-up of 9.9 years (all-cause mortality 30.0%). Higher SDMA independently predicted all-cause and cardiovascular mortality, was inversely associated with cholesterol efflux (p = 0.004), and reversed the protective effect of HDL-C on mortality. Replicated in 1424 MONICA/KORA S3 participants, the findings identify SDMA as a marker of HDL dysfunction in chronic kidney disease.
