The UGT1A1*28 promoter variant (rs3064744) and baseline bilirubin levels were examined as mortality predictors in 3,316 LURIC participants followed for a median of 10.4 years, during which 995 (30.0%) died. Multivariate analysis showed UGT1A1*28 was associated with lower overall mortality (HR 0.86; 95% CI 0.78-0.95; p = 0.003), whereas higher bilirubin predicted increased mortality (HR 1.014; p = 0.019). The findings suggest UGT1A1*28 confers protection through mechanisms beyond bilirubin metabolism.
