NT-proBNP and Big-Endothelin-1 (Big-ET-1) were assessed as diagnostic markers for heart failure with preserved ejection fraction (HFpEF) in 439 LURIC patients. NT-proBNP required GFR-dependent cut-offs (250 pg/mL without CKD; 750 pg/mL when GFR < 60 mL/min/1.73 m2), whereas Big-ET-1 at 0.85 fmol/L distinguished HFpEF from non-HF patients regardless of renal function. These findings suggest that adding Big-ET-1 measurement improves HFpEF diagnosis in patients with concurrent chronic kidney disease.
