A Markov cohort model was applied to 1,530 LURIC participants with stable coronary artery disease to assess PCSK9 inhibitor cost-effectiveness. At 8,500 Euros per year and a 32.5% relative risk reduction, ICERs were 112,530 and 108,660 Euros per QALY in women and men, respectively, remaining below the 150,000 Euro threshold. Broader use would strain healthcare budgets, favouring a focus on very high-risk patients or substantial price reductions.
