Vitamin B12 concentration, relative telomere length (RTL), and all-cause mortality were examined in 2,970 LURIC participants. Both the lowest and highest B12 quartiles carried higher mortality risk than the mid-range (HR 1.23 and 1.24, respectively), and longer telomeres were associated with substantially lower mortality within each extreme group (HR 0.57 and 0.40). The findings suggest that homocysteine and inflammation drive mortality through distinct mechanisms in low- and high-B12 subjects.
