Six circulating markers of endothelial function were analysed in 1,926 LURIC participants stratified by smoking status over a median mortality follow-up of 10.6 years. Smokers had higher sICAM-1, sE-selectin and sP-selectin but lower sL-selectin than never-smokers; sL-selectin was inversely associated with mortality (HR 0.67-0.72 per 1-SD increase). Adding sL-selectin to traditional risk factors improved AUC from 0.725 to 0.752 (p = 0.034) with an NRI of 43%, supporting its potential as a cardiovascular risk marker in smokers.
