Cluster analysis of coronary angiographic reports from 4,344 patients at Heidelberg University Hospital identified four distinct CAD phenotypes, independently confirmed in 3,129 LURIC patients. The clusters differed in stenosis distribution, metabolic syndrome prevalence, inflammatory biomarker levels, acute coronary syndrome presentation, and overall cardiovascular risk. All-cause and cardiovascular mortality differed significantly between clusters, suggesting that CAD encompasses multiple pathophysiologically distinct disease entities.
