Galectin-3 bound more strongly to red blood cells and von Willebrand factor in non-O blood groups in vitro, and plasma galectin-3 was lower in non-O LURIC patients (n = 2571; A: 15.0, B: 14.9, AB: 14.0 vs. O: 17.1 microg/L; p < 0.0001), validated in PREVEND. Despite reduced levels, galectin-3 retained prognostic value for all-cause mortality across blood groups, suggesting blood group epitopes modulate its plasma concentration and biomarker performance.
