Branched-chain amino acid (BCAA) concentrations were examined in relation to mortality risk among 2,236 LURIC participants over a median follow-up of 10.5 years, during which 715 deaths occurred. After adjustment for confounders, higher BCAA levels were inversely associated with all-cause mortality (HR 0.75 per 1-SD increase in log-BCAA) and cardiovascular mortality (HR 0.72). These findings indicate that BCAAs are directly linked to metabolic risk factors yet paradoxically protective against mortality in persons with intermediate-to-high cardiovascular risk.
