Metabolic clustering was applied to 1,269 LURIC participants without diabetes and 794 with type 2 diabetes, with a median follow-up of 16.1 years. High-risk prediabetes clusters showed elevated all-cause mortality (HR 1.42-1.46), while T2D subtypes including severe insulin-resistant and mild age-related diabetes had HRs of 2.2 and severe insulin-deficient diabetes reached HR 2.5. These findings indicate that glucose-metabolism-based clustering stratifies survival risk even among patients undergoing invasive coronary diagnostics.
