A functional vitamin D metabolite profile (24,25(OH)2D <1.2 ng/mL and VMR <4%) was assessed in DESIRE (n=2010) and LURIC (n=2456). In LURIC, only 21.2% met the low-profile criterion versus 69.3% classified deficient by a conventional 25(OH)D cutoff; the low profile was associated with elevated PTH, accelerated bone turnover, and higher 10-year all-cause mortality. The functional approach more precisely identifies patients at clinical risk than a fixed 25(OH)D threshold.
