Carbamylated albumin (C-Alb) was quantified in 3197 LURIC participants referred for coronary angiography and followed for 10 years. Fourth-quartile C-Alb was associated with a hazard ratio of 2.52 (95% CI 2.11-3.01) for all-cause mortality, attenuating to 1.25 (1.02-1.53) after adjustment for cardiovascular risk factors including eGFR. The association was strongest for congestive heart failure death, with an adjusted HR of 3.99 (2.40-6.63) per 1-unit increase of log-transformed C-Alb.
