Serum uromodulin (SUmod) and carbamylated albumin (C-Alb) were assessed in 3316 LURIC participants for their combined predictive value for mortality. The C-Alb high/SUmod low group carried the highest mortality risk (HR = 3.30; 95% CI, 2.73-3.99), persisting after adjustment for eGFR (HR = 1.89). Adding both markers improved AUC for all-cause mortality from 0.728 to 0.751 and for cardiovascular mortality from 0.698 to 0.727, demonstrating complementary prognostic value beyond eGFR.
